Prior to joining forces in 2015 to establish Formé Medical Center and Urgent Care — a community-based healthcare facility — president/founder Gina Cappelli and CEO Maria Trusa had both achieved success in healthcare.

Cappelli had already opened Formé Rehabilitation in 2000 but shifted gears in 2014 to launch Formé Medical Center, which at the time included urgent care and physical therapy. Trusa came onboard in 2015, bringing valuable expertise from her role as executive director of the prestigious Scarsdale Medical Group. Under her leadership, services were expanded to include primary care, gynecology, podiatry, neurology, and cardiology.

Through their vision, teamwork, and dedication, Formé Medical today serves the needs of 6,000 insured patients. It also provides approximately 4,000 uninsured people with access to affordable, high-quality, dignified healthcare in a state-of-the-art facility located in White Plains.

Via Formé Medical Membership, which debuted in March 2016, the uninsured have unlimited access to medical services for as low as $1 to $2 a day. Cappelli and Trusa also cofounded Promise to Aid — a nonprofit entity that assists those who can’t afford preventive medicine, medication, or surgery.

They are the proud recipients of the Diversity Good Scout Award from the Westchester-Putnam Council, Boy Scouts of America, and the Westchester Community Care Award, which is presented by UnitedHealthcare. Trusa, an immigrant from the Dominican Republic, also received the Adelante Award, which recognizes the contributions of the Hispanic community. She serves as acting vice president of the Westchester Hispanic Chamber of Commerce and board member of the Westchester County’s Hispanic Advisory Board.
