A New, Innovative App Aids Patients With Simplified Care Coordination

The new myCareGPS app will provide patients with improved quality of care and better outcomes.

 

Patients increasingly depend on — and expect — quick access to ‘round the clock’ healthcare. In a bid to meet this need, ACCESS Nursing Services, the largest female-owned clinical nursing business in the state, is hoping to usher in a new era of interaction with their game-changing app, myCareGPS.

According to Louise Weadock, chief nursing officer and founder of White Plains-based Access Nursing Services, myCareGPS is tailored to address the shifting needs of both healthcare providers and patients. “The healthcare environment and the paradigm of reimbursements is changing,” explains Weadock. “Our goal is to get patients out of that acute-care hot zone and into transitional care so they don’t rubber-band back to the hospital. myCareGPS is a transitional and chronic care management cloud-based software that simplifies care coordination, while improving the quality of care and outcomes, managing cost, and generating new revenue streams.”

The app is a platform that enables the link between care coordination from healthcare institution to home. “It connects doctors, caregivers, patients, and families in real-time,” says Weadock. “Doctors can get a 360-degree [sense] of the patient and generate a new revenue stream for chronic care management without adding overhead or new patients.”

Weadock adds that myCareGPS leverages insights, best practices and communications protocols, and pulls discharge data from regional hospital centers and established care plan exemplars to ensure continuity of treatment by caregivers and the best possible care for the patient.

The app is powered by nurses, and it is already getting great reception in the market through its recognition by nonprofit health organizations. ACCESS plans to deliver its services to large health systems in the NY Metro such as NewYork-Presbyterian, Mount Sinai, Manhattan Jewish Health Services, and more.
